Registered Nurses (RN) Case Manager needed for a growing Home Health agency in Fort Worth, Texas. Must be motivated and caring. Job Duties Include: Provide skilled nursing care to geriatric patients in a home setting. Implement, develop, document the plan of care to ensure quality & continuity of care for patients. Provide education to patient and/or caregiver. Provide effective communication to patient, family and team members. Job Requirements:Must have an active nurse license in the state of TexasMinimum of 6 months of licensed hands on patient care in the past 24 months as a RN. Current CPR Benefits: Competitive Pay, Direct Deposit, Paid Training, Flexible Schedules. Experience preferred but willing to train the right person.
