Applicants seeking an exception from the English and US civics test requirements after applying for US citizenship based on physical or age-linked disability or mental deficiency which lasted for more than a year must submit the Form N-648 according to USCIS regulations.
Use Form N-648 to get a waiver from taking test after applying for US citizenship using N400 form (http://www.uscitizenship.info/us-citizenship/U-S-Citizenship-application-Form-N-400.jsp). For special cases the US immigration Form N-648 can be used to get exception from the citizenship test. Only if you are eligible you are granted exception.It should be submitted by a qualified physician.
