Complete a short 5-10 minute healthcare survey that pays $25. Medicaid/Medicare or employer provided plans don't qualify . The survey asks questions about purchasing health insurance and your experience with the insurance marketplace. You will also be required to submit a copy of your insurance card as proof that you have it. The survey is designed to be a referral survey as well, so you also get $25 for anyone you refer that is eligible and takes the survey - minimum $75 for payout of referral bonuses.
